  • LPN
    This position has primary responsibility for the health care needs of the residents of Abrams Assisted Living. We use PointClickCare for documentation and offer a relaxed and supportive work environment. Job Description DUTIES & RESPONSIBILITIES Health Services: Complete new/prospective resident assessments, and update annually or more often as changes in condition occur. Create new resident care plans, and update annually or more often as changes in condition occur. Provide staff training related to each resident’s care plan. Keep accurate records of all physician’s orders for medications and treatments. Ensure signed orders are completed for all medications. Work with pharmacies to have correct supply of medications on hand. Administer medications to residents. Provide staff training and testing on medication administration. Supervise those employees trained for medication administration and provide daily oversight and on-going training. Document all staff training regarding medication administration. Provide care to residents who need nursing services. Schedule and facilitate family/staff care planning meetings at least every 6 months or more often if needed. Document all nursing notes in the electronic medical record (PointClickCare). Assist with resident emergency situations. Miscellaneous: Work in coordination with the Director of Nursing, keeping them informed of the residents' needs and problems. Support and enforce infection-control policies and procedures. Participate in community emergency and safety programs. Provide input to the Director of Nursing on the evaluation of CNAs and Certified Medication Aides. Assist in reviewing facility policies and procedures. Participate in Monthly Quality Committee Meetings. Maintain professional conduct and adheres to all HIPPA regulations regarding confidentiality. Minimum Requirements for the LPN Role: Previous experience working within the senior living community. Ability to communicate effectively at a professional level in writing and verbally. Is resourceful and willing to satisfy both ordinary and extraordinary needs of prospective residents/families. Must be able to work flexible hours. Must be computer literate and be efficient in the use of electronic health records (EHR). Ability to use the internet for research and emailing. The minimum licensure and certification requirements of the position are: RN or LPN in the District of Columbia or eligibility for DC licensure. Current Cardiopulmonary resuscitation (CPR) certification. It is expected that every employee will seek out educational opportunities and take any necessary steps to stay current in their professional field of expertise.   • RN Supervisor
    This position supports the Health Services Director to run the department efficiently, ensuring that sufficient staffing is in place, providing resident care and medication administration, and provides care coordination among all providers. We use PointClickCare for documentation and offer a relaxed and supportive work environment. ____________________________________________________________ Full-Time 3 pm - 11 pm Mon-Friday (Participate in the Manager on Duty (MOD) rotation as scheduled for the community) ____________________________________________________________ Registered Nurse Supervisor Duties & Responsibilities Health Services: Support the Health Services Director (HSD) with New / Prospective Resident Assessments Support the HSD to update Resident Assessments Annually, or more often as changes in condition occur. Create New Resident Care Plans in PointClickCare in coordination with the Health Services Director. Review / Update Resident Care Plans Annually, or more often as changes in condition occur. Assists in reviewing facility policies and procedures. Participate in Monthly Quality Committee Meetings. In coordination with the Health Services Director and Executive Director, manage the annual regulatory survey process. Participate in the Manager on Duty (MOD) rotation as scheduled for the community Minimum Requirements for the RN Supervisor Role: Previous experience working within the senior living community.   • Trained Medication Aide | PRN
    Shift and schedule: PRN Job Description (The list below is a summary of this position's duties & responsibilities) Assist residents with all activities of daily living (ADLs) per schedule. Administers medications to residents in accordance with the physician orders, as documented on the medication administration record (MAR). Accurately records on the MAR medications administered, or withheld or refused and the reason why. Monitors the need for medication refills and communicates appropriately with the pharmacy and the Director of Nursing as necessary. Receives deliveries from the pharmacy for the Assisted Living building. Assists residents with ambulation, in and out of bed or chairs per care plans. First line of response to emergency call alarm. Records all relevant data in electronic medical records per facility policy. Assists residents with catheter care as indicated. Assist residents during admission, transfer and/or discharge processes. Provides end of life care as indicated. Assists with meal services, including setting up tables, serving meals and cleaning up the dining room. Assists in maintaining a clean, comfortable and safe environment by providing housekeeping services as needed. Completes scheduled and unscheduled laundry services for each resident. Assists with individual and group social / recreational activities; encourages resident participation in activities. Documents both scheduled and unscheduled care that is provided, in addition to incidents, behaviors, and out of the ordinary events, including a residents’ refusal of services in the electronic medical record.
