LPN/RN Staff NurseThis position has primary responsibility for the health care needs of the residents of Abrams Assisted Living. We use PointClickCare for documentation and offer a relaxed and supportive work environment. Job Description DUTIES & RESPONSIBILITIES Health Services: Complete new/prospective resident assessments, and update annually or more often as changes in condition occur. Create new resident care plans, and update annually or more often as changes in condition occur. Provide staff training related to each resident’s care plan. Keep accurate records of all physician’s orders for medications and treatments. Ensure signed orders are completed for all medications. Work with pharmacies to have correct supply of medications on hand. Administer medications to residents. Provide staff training and testing on medication administration. Supervise those employees trained for medication administration and provide daily oversight and on-going training. Document all staff training regarding medication administration. Provide care to residents who need nursing services. Schedule and facilitate family/staff care planning meetings at least every 6 months or more often if needed. Document all nursing notes in the electronic medical record (PointClickCare). Assist with resident emergency situations. Miscellaneous: Work in coordination with the Director of Nursing, keeping them informed of the residents' needs and problems. Support and enforce infection-control policies and procedures. Participate in community emergency and safety programs. Provide input to the Director of Nursing on the evaluation of CNAs and Certified Medication Aides. Assist in reviewing facility policies and procedures. Participate in Monthly Quality Committee Meetings. Maintain professional conduct and adheres to all HIPPA regulations regarding confidentiality. Minimum Requirements for the LPN Role: Previous experience working within the senior living community. Ability to communicate effectively at a professional level in writing and verbally. Is resourceful and willing to satisfy both ordinary and extraordinary needs of prospective residents/families. Must be able to work flexible hours. Must be computer literate and be efficient in the use of electronic health records (EHR). Ability to use the internet for research and emailing. The minimum licensure and certification requirements of the position are: RN or LPN in the District of Columbia or eligibility for DC licensure. Current Cardiopulmonary resuscitation (CPR) certification. LNA (Health Services Assistant)The Health Services Assistant’s primary responsibility is assisting residents with their personal care needs, activities, laundry, and meal service as needed. Job Description Assist residents with all activities of daily living (ADLs) per schedule. Receive and give shift report twice daily at beginning and end of shift. Assists residents with ambulation, in and out of bed or chairs per care plans. Provides Health Services Director and Care Manager with pertinent information for resident assessment and care plan completion. First line of response to emergency call alarm. Attends care conferences to review resident care plan as needed. Maintains an awareness of supply and equipment needs and reports needs to the Care Manager and Health Services Director. Records all relevant data in electronic medical records per facility policy. Report unusual or significant changes in resident appearance or behaviors to Health Services Director and Care Manager. Assists residents with catheter care as indicated. Assist residents during admission, transfer and/or discharge processes. Provides end of life care as indicated. Perform special treatments that fall within the scope of education and training as directed. Demonstrates knowledge of and follows infection control procedures. Assists with meal services, including setting up tables, serving meals and cleaning up the dining room. Assists in maintaining a clean, comfortable and safe environment by providing housekeeping services as needed. Completes scheduled and unscheduled laundry services for each resident. Assists with individual and group social / recreational activities; encourages resident participation in activities. Documents both scheduled and unscheduled care that is provided, in addition to incidents, behaviors, and out of the ordinary events, including a residents’ refusal of services in the electronic medical record. Housekeeping / Resident Services AssistantJob Description (The list below is a summary of this position's duties & responsibilities) Activities Assist the Resident Services Director to develop, implement, and publish a schedule of social activities to strengthen and enhance the social, physical, and mental skills of the residents. Supervise and participate in activities, which includes preparations, participation, clean up, and compliance with safety regulations. Handle incoming phone calls, greet and assist residents and visitors entering the building. Help the Resident Services Director to maintain a well-organized and labeled event decorations and supplies in storage. Respond to resident needs for assistance, as appropriate. Utilizes the electronic medical records system to track resident attendance to activities. Transportation Assists residents onto and off transporting vehicles. Accompanies residents to appointments outside of the assisted living as scheduled. Housekeeping Clean resident apartments as needed through each week. This includes bathrooms, kitchens, bedrooms and living/dining areas and floors. Dust and spot clean sofas/chairs daily as needed. Check for debris and vacuum as necessary. Sweep and mop dining room twice daily following breakfast and dinner meals. Identify potential maintenance issues and submit work orders. Responsible for obtaining cleaning supplies, stocking cart and maintaining clean, neat and orderly cart and storage area. Dining Services Set or reset dining room tables. Greet residents in the dining room and take accurate food and beverage orders. Serve complete meal to all residents in the dining room in a timely fashion. Perform side work correctly as assigned according to the side work procedure guidelines and to the level required on the sanitation checklist. Medication AssistantJob Description (The list below is a summary of this position's duties & responsibilities) Assist residents with all activities of daily living (ADLs) per schedule. Kitchen AssistantThis position is responsible for the performance of general kitchen cleaning duties, inclusive of dishwashing and daily floor cleaning in the kitchen. The position may also be asked to assist with food preparation. Performance of dishwashing procedures using automatic dishwasher and manual pot and pan sink. Assisting in food preparation as directed. Reports any safety hazards immediately. Participates in required staff training. Maintains professional confidentiality regarding residents and community. CookThis position is responsible for the preparation of nutritious, attractive meals and ensures cleanliness of the food production and service areas. Job Description (The list below is a summary of this position's duties & responsibilities) Prepares and serves meals as indicated on planned menus, using standardized recipes, portion sizes, and approved substitutions. Ensures that meals are served at scheduled times, planning and organizing food preparation tasks appropriately. Prepares meals for modified or therapeutic diets, according to physician orders and modified menus. Is responsible for clean-up duties as needed after each meal service. Assures that resident food preferences are met. Observes and reports to the Dining Services Director any problems, concerns or issues regarding the dining service department. Visits with residents in the dining room to assure resident satisfaction and obtain feedback. Offers support and assistance to other departments as directed and needed. Maintains organization and cleanliness of work areas. Participates in required staff training. Must be able to read, write, speak, and understand the English language. Experience cooking in a commercial kitchen. Formal culinary educational training at a recognized school or university preferred.
